In order to make sure that I get the history of your problem correct, I ask that you fill out three pages of information. I have designed them myself to be as quick and painless as possible. I hate forms, too, but this information will help me to help understand your problem.

1. Click the Go to Forms button below. Three pages of forms are displayed.

2. Print the forms and complete them at home. If your hand problem makes it difficult for you to fill in the forms, wait until you get to the office and ask my secretary to help you.

Note: If you have the full Adobe Acrobat product, not just Acrobat Reader, you can save the forms to your hard disk, open them in Adobe Acrobat, and fill in the forms your computer. Print the completed form and go to step 3.

3. Bring the forms with you to your appointment. Do not email the forms to me. This web site is not equipped to receive the forms electronically.
